Home Choice helps children and youth transition from a hospital or residential treatment center who have had lengths of stay of at least 90 days and have had Medicaid, back to a home and community based setting.
A transition coordinator will assist the guardian and youth by providing supports for the child or youth to successfully return home. These supports may include community services, locating housing, equipment, clothing, household items, independent living skills training and transportation. The coordinator can work prior to and following discharge from the facility to help coordinate having the right supports in place for the family.
